On-Ice Volunteer Form

Form must be completed by ALL On-Ice Volunteers that are not registered bench staff. Maximum 2 per team. DEADLINE: November 16, 2025 at 9PM

Minimum age to volunteer is 2011 birth year.

On-ice volunteers are not permitted on the bench during games.

ON-ICE VOLUNTEERS BORN 2011-2008

Volunteers must be a full 2 years above the division they are volunteering for and must wear Certified Hockey Gloves, Helmet and Skates as per Hockey Canada. **Volunteers in this age group must already be OMHA insured in order to help on the ice.**

ON-ICE VOLUNTEER 18 YRS AND ABOVE

Volunteers 18 yrs and older must submit a PRC to OHF. Insurance must be purchased by OMHA in order to help on the ice. Volunteers must wear Certified Hockey, Gloves, Helmet and Skates as per Hockey Canada.
